1. The alarm was set wrong

The most common cause is the least satisfying: 7:00 PM instead of 7:00 AM, or a repeating alarm that skips the day you needed. Open Clock → Alarms and check the time, the AM/PM, and which days the repeat covers. A weekday alarm does nothing for you on Saturday, and a Saturday flight does not care.

4. You dismissed it in your sleep

This is the cause nobody wants to accept, and it is more common than every technical cause combined. Half-asleep, you can find the phone, stop the alarm, and return to sleep with zero memory of doing it. The alarm "didn't go off" — except it did.
